I confess to not having known the printing/destroying history to this; but seems like if someone found a copy from which to republish; they would have also found the index. Tends to be a bit on the 1960's liberal slant; but the analyses were generally valid. Some of the reviews of the late 1960's era were prone to psycho-babble; but discussions of post-war events up to about 1965 were quite good. Seemed to do a good job of giving the behind-the-scenes insights into events.
